Ages 17 and under
Two physical forms of Government issued ID for client
1. A physical form of Government Issued Photo ID provided by parent/legal guardian*
*Grandparents, Aunts, Uncles, Siblings, etc. must provide physical proof of being legal guardian otherwise they are not eligible to provide consent
2. A physical copy of clients Student Photo ID paired with matching Government Issued ID is accepted
Example: Physical student ID with Health Care Card is accepted
CHILD ID REQUIREMENTS(6-12YRS)
BIRTH CERTIFICATE & AHC IS ACCEPTABLE
Ages 16 and up
• Client may sign for themselves with a physical form of
Government Issued Photo ID
*Parent/Legal Guardian is not required to provide consent
Ages 18 and up
• A physical form of Government Issued Photo ID
*EXPIRED ID WILL NOT BE ACCEPTED*
2*DIGITAL PHOTOS OF ID’S MAY BE ACCEPTED BY PIERCERS
